Digital Therapeutics: Can Apps Become Part of Pediatric Care?

What Are Digital Therapeutics?
Digital therapeutics are software-based interventions designed to prevent, manage, or treat medical conditions. Unlike most health apps, they are developed using scientific evidence and, in some cases, undergo rigorous clinical testing before receiving regulatory authorization.
Some digital therapeutics are FDA-authorized, meaning they have been evaluated for safety and effectiveness for specific medical conditions. These tools are prescribed or recommended by healthcare providers and are intended to complement, not replace, clinical care.
The global digital therapeutics market is expanding rapidly. Industry reports estimate it could exceed $30 billion by the early 2030s, driven by advances in artificial intelligence, mobile health technology, and personalized medicine.
FDA-Authorized Digital Therapeutics
One of the most significant milestones in pediatric digital health came when the U.S. Food and Drug Administration authorized EndeavorRx, the first prescription video game approved to help improve attention function in children aged 8–17 years with primarily inattentive or combined-type ADHD.
Unlike traditional video games, EndeavorRx was designed using neuroscience principles to stimulate attention networks in the brain. Clinical studies demonstrated modest but measurable improvements in attention when used as part of a comprehensive ADHD treatment plan.
The approval marked an important shift in pediatric healthcare: software could become part of evidence-based treatment alongside behavioral therapy, education, and medical care.
Can Technology Improve Cognitive Skills?
Researchers are exploring whether carefully designed digital programs can strengthen specific cognitive abilities, including:
- Attention
- Working memory
- Processing speed
- Cognitive flexibility
- Executive functioning
Some cognitive training platforms have shown promise for targeted skills, particularly when used under professional guidance. However, research also suggests that improvements in one task do not always transfer to broader academic performance or everyday functioning.
In other words, digital cognitive training may support certain abilities, but it is not a shortcut to improving intelligence or replacing comprehensive developmental interventions.
Technology and Speech-Language Development
Technology has also transformed communication for many children.
Speech-language pathologists increasingly incorporate digital tools such as:
- Augmentative and Alternative Communication (AAC) apps
- Picture-based communication systems
- Interactive language-learning platforms
- Vocabulary-building activities
- Articulation practice tools
For children with limited verbal communication, AAC technology can provide a reliable way to express wants, needs, thoughts, and emotions. Research consistently shows that appropriately introducing AAC does not delay spoken language. In many cases, it supports communication development and reduces frustration by giving children an effective means to interact with others.
Separating Evidence From Hype
The digital health marketplace now contains thousands of apps claiming to improve attention, speech, learning, autism, or brain development. Yet only a small proportion have been scientifically evaluated.
Parents should ask several important questions before investing time or money:
- Has this technology been studied in clinical research?
- Is there published evidence supporting its effectiveness?
- Was it developed with healthcare professionals?
- Is it appropriate for my child’s specific needs?
- Has my child’s healthcare team recommended it?
If the answer to these questions is unclear, caution is warranted.
A well-designed app may be helpful. An unsupported claim is not the same as scientific evidence.
Technology Works Best as Part of Comprehensive Care
One of the biggest misconceptions about digital therapeutics is that they can replace therapists or healthcare providers.
They cannot.
Children learn through relationships, play, movement, communication, and real-world experiences. Technology is most effective when it supports these interactions rather than replacing them.
For example, an occupational therapist may recommend a digital tool to reinforce attention skills at home, while a speech-language pathologist may incorporate AAC technology into communication goals. In these situations, technology becomes one part of a coordinated, family-centered care plan.
Looking Ahead
Artificial intelligence, wearable devices, virtual reality, and digital therapeutics are likely to play an increasingly important role in pediatric healthcare over the next decade. These innovations have the potential to improve access to care, personalize interventions, and help clinicians monitor progress between appointments.
However, technology should always serve children—not the other way around.
